Holiday Form

Hi All

I am looking to implement a holiday booking form but i would like it to be able to do the following things.

1 automatically enter the name of the person onto the form

2 have an email sent back to the user to tell them how much holiday that they have left.

i am new to sharepoint and any information on how to do this would be greatly appreciated
