Checkbox in Custom List

Hi All,

I have created one custom list as i shown in attachment in that i have customize one checkbox,label and button. On button click i want the selected checkbox. And if checkbox is selected then i want to take the label value which i have added after the checkbox.

So can anybody help me how to get the value of label which checkbox is selected.
