Sharepoint Designer is the easiest way to build custom workflow solutions. However, you'll probably also find that it's remarkably easy to manipulate the OOTB WFs. The Three State Workflow, for example, isn't a feature turned on with stardard sites, and I've found it fits many situations without having to turn to SPD.
More expensive, but MUCH more powerful, are WF solutions like k2 ($$$$$$), Skelta ($$$), Caparis ($$$$), etc. Or, if you're really smart AND really bored, the WF tools for VS.NET. ;-)

Simply put, you have two approaches. Number one: use SPD, it is a good platform for you to customize workflow as long as you get the sufficient knowledge and time. Number two: get a third party tool which already did those. there are various tools like this. But I not know is there anyone that fit you.

Well there are also Nintex products that are superb to handle workflow and even create them outside SharePoint. I guess there is always a third option as well. Develop your own workflow inside Visual Studio or even cooler, create your own Custom Activities inside Visual Studio for use inside SharePoint Designer.
